چکیده

background exertional heat stroke (ehs) is still a main cause of death in sport. many of ehs complications could have been prevented if ehs had been recognized and treated early and properly. case presentation we report an unusual case of multiple organ failure caused by ehs due to intensive sportive activities in a hot environment with lack of primary care. a 35-year-old healthy sportive man was admitted in our hospital because of muscle aches and weakness as well as dark urine three days after a six hour marathon run (agadir semi-marathon) in a very sunny day. patient developed rhabdomyolysis, acute renal failure (arf) requiring hemodialysis because of hyperkaliémia, azotémia and severe metabolic acidosis, disseminated intravascular coagulation and acute liver failure. unfortunately, after eight days of intensive care, the patient died from septic shock and multiple organ failure conclusions this case reminds us that, despite the advancements of knowledge in the area of ehs prevention, recognition, and treatment, knowledge has not been translated into practice.

Multiple organ failure caused by non-exertional heat stroke after bathing in a hot spring.

Heat stroke is a life-threatening illness, and the disease spectrum can include the involvement of multiple organs to varying degrees. Rhabdomyolysis with renal function impairment is frequently noted in this disease. However, acute hepatic failure has been rarely reported in non-exertional heat stroke. We report a case of acute hepatic failure combined with disseminated intravascular coagulopa...

Acute Hepatic Failure as a Leading Manifestation in Exertional Heat Stroke

Background. Acute hepatic failure (AHF) is uncommon as a leading symptom in patients with exertional heat stroke (EHS). Which stage to perform the liver transplantation for severe hepatic failure in EHS is still obscure at clinical setting. The conservative management has been reported to be successful in treating heat-stroke-associated AHF even in the presence of accepted criteria for emergenc...

Exertional heat stroke in a marathon runner with extensive healed deep burns: a case report

Exertional heat illness typically occurs over hours in younger athletic patients or military recruits who exercise at elevated temperatures for a sufficient period of time to cause the rate of heat production to exceed the capacity of the body to dissipate heat. Since the physiological response to exercise includes cutaneous vasodilation and sweating, any limitation of such a response can cause...

Severe heat stroke with multiple organ dysfunction

Second, why were conventional cooling techniques terminated after 8 hours when the graph (Figure 1 in the report by Broessner and coworkers [1]) indicates that body temperature was effectively lowered to below 39°C (the initial goal in treatment of heat stroke)? We question the cooling technique, because aggressive conventional cooling in the intensive care unit should achieve 39°C within 1 hou...

Exertional heat stroke: new concepts regarding cause and care.

When athletes, warfighters, and laborers perform intense exercise in the heat, the risk of exertional heat stroke (EHS) is ever present. The recent data regarding the fatalities due to EHS within the confines of organized American sport are not promising: during the past 35 years, the highest number of deaths in a 5-year period occurred from 2005 to 2009. This reminds us that, regardless of the...
